Once you have completed filling out each form, Please feel free to email them to [email protected] prior to your appointment for review. Or you may bring your completed forms to your first appointment and arrive 15 minutes early for review to take place prior to your appointment. Thank you.

Cancellation Policy
We have a 48 business hour cancel policy that starts April 1, 2016. If you cancel your appointment in less than 48 business hours, you will be charged the full fee of your appointment at your next visit. You will receive an automated reminder in advance of your appointment to help avoid such an occurrence. We find that our firm 48 business hour policy is a simple and basic courtesy to our schedule as well as yours. 

Product Return/
​Exchange Policy
Absolutely NO returns or exchanges on any product will be made, all sales are final. As we are unable to control the temperature of these products once they leave our office and cannot guarantee their quality once returned. 

Punctuality Policy
Our priority is to spend quality time with each and every client of Opulent Natural Health. In order to accomplish this, we schedule the appropriate time needed for each client. If you are late to your appointment, you will be responsible for the full cost even if your appointment is shortened. Please leave a generous amount of time for traffic irregularities and feel free to call to inform us if you are running late.
